Commissioning Strategy for People With Long Term Conditions Aged Between 18 and 64.

At the Council, we have recently published our commissioning strategy for people with long term conditions. The strategy sets out the direction for services and support for adults from 2008 to 2013.

A commissioning strategy looks at individual needs and the needs of the community as a whole. We can then look at how we can support people, for example, by providing services to meet those needs.

In developing this strategy, we involved a wide range of people who use services, carers, voluntary & independent organisations and BFBC staff.   In the first stage people told us what they thought about existing services and where the gaps were. We then consulted widely on these findings to make sure that we heard all the important messages.

We also looked at the population of Bracknell Forest and how this may change over the next 5 years. We considered how many people are likely to experience a long term condition over the next 5 years and thought about how we could reduce some of the risk factors. The money available for support and services was also considered.

By gathering all this information, we were able to assess the needs of people in Bracknell Forest and plan for the future.
