Intermediate Care Services
Rehabilitation for clients living in the Borough of Bracknell Forest is provided by services working as part of Bracknell Intermediate Care.
Intermediate Care Services have been developed in Bracknell Forest primarily to support and enable people to remain in their own homes for longer and to facilitate timely discharge from hospital back to individuals' usual places of residence.
Rehabilitation is wide and encompassing, as is our approach to optimising individuals' health and well being in partnership with those we aim to empower and enable.
Following risk assessment, which consists of a partnership approach with the individual and their carers, a risk management strategy will be agreed. This risk management will incorporate a balance between the individual's informed choices and our duty of care to them.
The aim of the service is to:
- Promote independence and maintain individuals in the community by providing short-term rehabilitation programmes to maximise individual potential
- Prevent inappropriate hospital admission and re-admission
- Facilitate timely, safe and effective discharge from Acute Care
- Maximise individuals’ independence and thereby reduce the need for long term care and support
- To reduce the need for long term social support and minimise the need for long term placements
- To contribute to a reduction in the number of individuals falling by providing comprehensive assessment and appropriate referrals
These services can be delivered in a range of settings including at home, in a rehabilitation unit or at a community hospital.
